my fax machine fails to send the application form

Hi there.
I tried to send my shell accounts application form by fax, but my fax machine failed with errors permanently.
Can I scan the form and send it to you via email or some file sharing service. I can also send the hardcopy,
but it will take about two weeks (Russian Post is really slow).

Sure... a scanned signed application form is also accepted.
